GP Engage

Building strong relationships with primary health carers in our community

GP Engage

South Metropolitan Health Service (SMHS) delivers healthcare services to nearly a quarter of WA’s population via:

  • Fiona Stanley Fremantle Hospitals Group (external site)
  • Peel Health Campus (external site)
  • Rockingham Peel Group (external site)
  • Community-based services
  • SMHS Mental Health.

In delivering these services, we work closely with general practitioners (GPs) across our catchment (external PDF 2MB) to:

  • improve patient care as patients transition between hospital and community care
  • provide excellent service delivery
  • strengthen communication pathways to support safer, more coordinated care.

GP Engage events provide valuable opportunities for local primary care professionals such as GPs and nurse practitioners to:

  • learn about SMHS-wide and hospital priorities, processes and programs affecting shared care
  • provide feedback and build stronger relationships with hospital medical staff. 

The events blend education, Q&A sessions and networking opportunities. Continuing professional development (CPD) content (self-logged) may be recorded after each event, with an attendance certificate provided.

Upcoming events

Shared care, shared solutions: GPs and hospital clinicians in partnership across Rockingham Peel Group (16 October 2025)

Join Executive Director of Rockingham Peel Group (RkPG) Dr Asheila Narang, Acting Director of Clinical Services Dr Helen Thomas and various heads of department as they provide updates on:

  • hospital service capabilities at Rockingham General Hospital
  • emergency medicine: the good, the bad and the ugly
  • discharge planning resource document
  • general medicine services – this will be an interactive session, where we discuss some of the challenges GPs are experiencing in this space and suggestions on how we can support them and our shared patients better.

Further details will be shared closer to the event date.

Participants can earn up to 3 self-logged CPD hours.

SMHS care in the community (March 2025)

This event focused on impactful programs delivered by SMHS Community Services to patients within the SMHS, East Metropolitan Health Service and North Metropolitan Health Service catchments.

SMHS offers a range of sub-acute allied health assessment, rehabilitation and care coordination services for patients recently discharged from hospital. Services are conveniently provided either in the patient’s home or at community venues throughout the metropolitan area.

Care of older adults across FSFHG (November 2024)

This event looked at the patient journey for older adults receiving care from FSFHG – from the community, into tertiary care and then returning to the community. Event highlights included:

  • insights into the patient journey, from admission to discharge
  • best practice in the care of older adults

Spotlight on FSFHG mental health services (August 2024)

Mental health services at Fiona Stanley and Fremantle hospitals provide evidence-based care with strong consumer and carer engagement to youth, adults and older adults.

Working collaboratively with GPs and community managed organisations is critical to supporting the long-term recovery of our patients. This event:

  • focussed on the various mental health services available at Fiona Stanley Hospital (FSH) and Fremantle Hospital, including their community-based services which accept GP referrals
  • included an educational component focused on best practices for prescribing medication to patients with mental health illnesses
  • provided GPs an opportunity to receive training for the Initial Assessment and Referral Decision Support (IAR-DST) tool.
